Financial Assistance
The FINANCIAL ASSISTANCE PROGRAM (FAP) is based on need. It is not an academic scholarship. The award monies are used to pay or defray the training and tournament costs of players demonstrating a legitimate need for financial assistance. FAP funds don't go to the individual player; rather, they go to the team and reduce the amount that the particular player needs to pay toward their training fee.
There are (3) three application file attachments supplied here:
1) Part A is to be completed by the coach or manager, one for each team. He or she needs to fill in all the team-specific information entirely and accurately, along with the names of those players on the team who they expect to apply for financial assistance, with any notes or information they may have with respect to the needs of such players
2) Part B (English) must be completed by each of the individual players applying for assistance. The team will not receive the financial assistance with respect to a particular player if the player or his family fails to submit Part B and the evidence of need required as set out in Part B. Part B of the application and any documents submitted in support of Part B will be kept confidential by the FAP review panel to the extent permitted by law. In fact, you and other team representatives should NOT see Part B or any of the supporting documentation. Rather, this should all be sent directly to the address indicated below.
3) Part B (Spanish) same as above only in Spanish for those who have difficulty with completing the English form. Only one Part B needs to be submitted per player. However, players that do not submit a Part B either in English or Spanish cannot be considered. It is important to note that a player that submits a Part B can be consider even if he is not listed on the Coaches Part A but we must have a Part A from the Coach so we have the team information to make a proper award.

For an individual to be considered for financial assistance this spring, both parts of the application must be submitted to me by October 15, 2011. To expedite this process, I ask that all coaches to complete and submit Part A for each of his teams to the extent they can in advance of that date.
Players and their families should understand that submitting an application DOES NOT relieve a player or his/her family of the requirement to pay teams training and other fees. Rather, they will be relieved of this obligation only if and to the extent that they are approved for a FAP award and the team receives money in respect of that particular individual.
For teams U12 and older: Please note that players will be acknowledging that they will be expected to perform volunteer services by accepting the financial award and that, in all likelihood, you will be asked to help out with the players' volunteer service opportunities.
